2024-2025 New Student-Athlete Checklist
All new WPU student-athletes must complete the following items. All WPU forms need to be completed via SportsWare Online before you report to campus in August.

NAIA
- Register for the NAIA Eligibility Center
- If you are a first-time student athlete in the NAIA, you must register with the NAIA Eligibility Center in order to play. You may not participate in intercollegiate contests until you receive full clearance from the NAIA Eligibility Center.
- FRESHMEN - Submit your final high school transcript and test scores to the NAIA
- TRANSFERS - Submit your final college transcript (after all coursework is complete) to the NAIA
- Your transcript may have been sent to the NAIA when you first registered, but a new one must be sent now that you have completed all of your high school or college requirements. This requires work on your part. Remember, not only are you relying on your school to send the transcript, but you're also relying on the NAIA to review your courses and make their final decision. Waiting until August to get your transcripts and test scores to the Eligibility Center is not an option. Start this process now!
- Medical History Form
- Student-Athlete Physical Form (This must be filled out by doctor performing physical)
WPU Athletics requires all new student-athletes complete a physical prior to participation. If you have not had a physical in the last six months, you will need one in order to practice or compete. Please print this form and take it with you to your physician's office. It must be signed by a physician, physician's assistant, or nurse practitioner - a WPU Physical form signed by any other medical professional (i.e. chiropractor) will not be accepted.This form must be completed before you will be allowed to practice or compete.
- Medical Insurance/Emergency Contact Information Form
The WPU Student-Athlete Medical Insurance Policy at students be covered by health insurance to be actively enrolled. Athletes must carry a primary insurance policy that will cover athletic injury occurring during any athletic event, practice, game, or gathering. Proof of major medical insurance is required for all student-athletes to be on file with the WPU Athletic Trainer's office prior to participation in practice or competition. Make a copy (front and back) of your insurance card and submit it with the form.
Dental injury claims and deductibles are also the responsibility of the student-athlete; however student-athletes are not required to carry dental insurance to participate.
You may not practice or play with your team until proof of medical insurance has been provided to WPU and your Medical Insurance Coverage form has been signed.
